By proceeding, I acknowledge and agree to the following:

• All Mediplan Diaspora Health Plans are annual healthcare service contracts, regardless of whether I choose to pay monthly, quarterly, or annually.

• If I select monthly or quarterly payments, my card will be automatically charged on a recurring basis until the end of the 12-month contract.

• Any attempt to cancel the plan before the end of the contract year may attract a penalty fee or result in forfeiture of unused balance, depending on the situation and usage stage.
